Distal humerus fractures in elderly patients have a high risk of complications when treated with open reduction internal fixation. Total elbow arthroplasty provides the most predictable functional outcome and avoids the risk of fixation failure in comminuted, osteopenic bone. However, great care is needed to minimize the risk of complications. Triceps insufficiency can be avoided with triceps-sparing techniques, and multiple techniques can reduce the risk of infection and wound complications.
