Presentation of a 22 year-old male with a distal third tibial shaft fracture treated with an intramedullary nail using semi-extended positioning and a suprapatellar entry. We provide a brief review of the indications and contraindications of tibial intramedullary nails as well as the goals of surgery. A step-wise technique video is then performed.
