ORIF of a Posteromedial Tibial Plateau Fracture Through the Lobenhoffer Approach

The posteromedial fragment is a well-described component of many fractures of the tibial plateau that can been seen either in isolation or as part of a bicondylar fracture. Optimal management of this fragment requires recognition on pre-operative imaging followed by open reduction and internal fixation. Lobenhoffer described a posteromedial approach to the proximal tibia that allows for direct visualization of the posteriomedial fragment and placement of an antiglide plate. We describe this approach with focus on patient positioning, pertinent anatomy, fracture reduction and implant placement.
