This video demonstrates the approach, reduction, and internal fixation of an intra-articular supracondylar distal femur fracture with a Hoffa fracture using a distal femoral locking plate. Direct visualization of the comminuted distal femoral articular surface through a limited approach without stripping of soft tissues at the fracture site is stressed. The authors emphasize the importance of an accurate intra-articular reduction followed by submuscular plate application with distal locking screws. Using an "outrigger" device proper plate application and percutaneous screw fixation in the diaphysis , to ensure appropriate alignment in both the coronal and sagittal planes, is demonstrated.
