Exposures Upper/Deltoid Split: Approach

This video discusses the lateral approach to proximal humerus. This approach is used for the management of proximal humeral fractures. It is especially useful for osteosynthesis of fractures of the greater tuberosity. The operative plan includes a skin incision from the lateral border of the acromion 5 cm distally, parallel to the axis of the humerus. Deltoid is split in line with its fibers no more than 5 cm distal to the lateral edge of acromion (to protect the axillary nerve). After surgery the wound is irrigated, and the closure of deltoid fascia, subcutaneous tissues and the skin is done.
